Make a Difference Beyond the Office!
Are you passionate about helping individuals and families overcome challenges, access resources, and build healthier, more stable lives?
At FamilyCare Behavioral Wellness Center, we believe meaningful behavioral health treatment extends beyond the counseling office. Our comprehensive programs bring together therapy, Targeted Case Management (TCM), family support, and community resources to help children, adolescents, adults, and families achieve lasting progress.
We are seeking a motivated, organized, compassionate, and dependable Targeted Case Manager (Behavioral Health Care Navigator) who enjoys working directly with people, building community partnerships, solving problems, and making a measurable difference.
This is an excellent opportunity for someone who thrives in a fast-paced environment, values teamwork, and takes pride in managing their responsibilities from beginning to end.
If you are someone who takes initiative, follows through, stays organized, and genuinely enjoys helping people navigate life's challenges, we would love to meet you!
What Is a Targeted Case Manager?
A Targeted Case Manager is an essential member of our behavioral health treatment team who helps clients and families identify their needs, access community resources, coordinate services, and overcome barriers affecting their mental health, daily functioning, and overall well-being.
Unlike a traditional office-only position, this role combines direct community engagement with structured office-based case management responsibilities.
Our TCM services are provided through Kentucky Medicaid and require consistent client engagement, individualized care planning, accurate documentation, timely follow-up, and compliance with program requirements.
You will work closely with clients, families, therapists, schools, community agencies, and other professionals to help ensure everyone is working toward the same treatment goals.
This is not simply a referral position or a job where you check in with clients once a month. It is an active, hands-on care coordination role that requires initiative, problem-solving, accountability, and meaningful engagement.
A Day in the Life: Office + Community
In the Office — Organization, Planning & Coordination
A significant portion of your workday involves managing your caseload and ensuring services are coordinated effectively. Responsibilities include:
Scheduling appointments, client contacts, home visits, and treatment-team meetings.
Making and returning phone calls to clients, guardians, providers, and referral sources.
Completing case notes, assessments, individualized TCM care plans, safety plans, and other required documentation.
Monitoring authorization dates, upcoming deadlines, required contacts, and outstanding documentation.
Tracking client engagement, missed appointments, referrals, and follow-up needs.
Coordinating with therapists and other members of the treatment team.
Communicating with schools, healthcare providers, community partners, and referral agencies.
Preparing for case reviews, service-team meetings, and discharge planning.
In the Community — Making a Direct Impact
You will also spend time in the field working directly with clients and families through:
Scheduled home visits and community-based meetings.
Face-to-face client and family contacts.
Identifying barriers related to housing, transportation, food security, education, healthcare, and family stability.
Connecting families with community resources and helping them navigate services.
Collaborating with schools, community organizations, and healthcare providers.
Supporting families involved with child protective services, foster care, or family court systems.
Participating in treatment-team meetings and care coordination activities.
Monitoring progress toward individualized goals and helping clients overcome barriers.
You must be comfortable transitioning between office responsibilities and fieldwork while managing multiple priorities throughout the day.
Your Key Responsibilities
1. Caseload Management & Client Engagement
Independently manage an assigned caseload of approximately 25 clients, with assignments adjusted according to program needs.
Establish and maintain professional, supportive relationships with clients and families.
Complete meaningful and consistent client contacts based on individualized needs and program expectations.
Schedule and conduct face-to-face meetings, home visits, community contacts, and care coordination activities.
Monitor treatment engagement and proactively address missed appointments, barriers, and nonparticipation.
Follow through on identified needs rather than simply providing a referral and closing the task.
2. Assessments, Care Planning & Documentation
Complete comprehensive TCM strengths-and-needs assessments.
Develop individualized TCM care plans with measurable goals, objectives, and interventions.
Assist with safety planning and coordinate appropriate responses to identified concerns.
Conduct care-plan reviews and monitor progress toward established goals.
Maintain accurate, timely, and clinically meaningful documentation in the electronic health record.
Complete all required forms, service reports, assessments, and authorization-related documentation.
Maintain organized tracking systems for contacts, appointments, care-plan deadlines, authorizations, and follow-up tasks.
Ensure documentation supports medical necessity and applicable Kentucky Medicaid requirements.
3. Care Coordination & Community Collaboration
Collaborate closely with assigned therapists, clinical supervisors, and other treatment-team members.
Coordinate services with community providers, healthcare organizations, and referral sources.
Maintain professional communication with Kentucky DCBS/CPS, attorneys, family courts, foster care providers, school personnel, and community partners, as authorized and appropriate.
Facilitate treatment-team meetings, case reviews, and interdisciplinary communication.
Advocate for appropriate resources and services while maintaining professional boundaries and confidentiality.
Coordinate transitions, referrals, aftercare planning, and appropriate discharge services.
4. Compliance, Productivity & Accountability
Maintain accurate records of required monthly contacts and completed services.
Monitor caseload activity, treatment deadlines, authorizations, and documentation requirements.
Complete case notes and other required documentation within agency timelines.
Remain responsive to client needs, team communications, and supervisor requests.
Meet established productivity, engagement, and quality expectations.
Identify concerns early, communicate barriers, and take initiative to resolve outstanding tasks.
Participate in supervision, training, quality improvement, and team meetings.
Assist with crisis coordination and safety-related concerns within the scope of the position and agency protocols.
What Makes Someone Successful in This Role?
We are looking for someone who brings both compassion and accountability to their work.
The ideal candidate is:
Highly organized: You can manage multiple clients, deadlines, appointments, and documentation requirements without losing track of important details.
Dependable: You follow through on commitments, maintain consistent attendance, and take responsibility for your work.

Adaptable: You can pivot when a client cancels, a family experiences a crisis, or an urgent need arises.
An excellent communicator: You are professional, responsive, and comfortable communicating with families, attorneys, schools, child welfare agencies, and other providers.
Detail-oriented: You understand that documentation, authorizations, and deadlines are essential components of quality care.
A strong time manager: You can balance client appointments, travel, phone calls, meetings, and paperwork efficiently.
A team player: You collaborate with therapists and colleagues rather than working in isolation.
Solution-focused: You enjoy finding resources, addressing barriers, and helping families navigate complex situations.
Flexible and committed: You understand that some days require adjusting priorities or staying beyond your usual schedule, when authorized and compensated as applicable, to complete time-sensitive responsibilities.
Professional: You maintain appropriate boundaries, confidentiality, sound judgment, and a respectful demeanor in challenging situations.
This position is best suited for someone who enjoys wearing multiple hats, can work independently, and takes ownership of their caseload.
